Capital Caring Health’s home-based primary care services offer elderly patients a chance to enjoy the comfort of their own homes while still receiving the treatment and assistance they need to manage pain even in times of illness.
Features of our primary care at home services include the following:
- House Calls: By doctors, nurse practitioners, and social workers who specialize in caring for older adults with advanced illness.
- Mobile Technology: We offer in-home diagnostic tests (blood work, X-rays, ultrasound) and an electronic portal for communication with our team.
- 24/7 Coordination of Care: Our team is available 24/7 by phone. We arrange primary and urgent care, specialists, hospital, and support services needed to age at home with dignity.
- Family Time: We work closely with families so all understand the care plan.

Payment
The cost of care for Capital Caring Health’s primary care at home services are generally covered by Medicare and other health insurance; that includes coverage for doctor and NP house call visits. Standard co-payments apply.
